On Duty

Below please find important reports/forms which assist the Residence Life Staff in dealing with situations promptly and appropriately. When submitting information, please be as accurate and descriptive as possible. Also, please follow the breakdown below of guidelines for these forms to ensure proper follow-up.

Duty Log
To be submitted following completion of final rounds. If a situation should occur after submission of the duty log, please inform your direct supervisor via email. The email would include but not describe the type of situation. Additional reports/forms should be submitted to your direct supervisor, the next morning, describing what took place. Overall, keep in mind that the duty log serves as a summary of what took place during duty and as a report on a building's condition.

Crisis Report
To be submitted following the incident. The crisis report focuses on the individual affected by an uncontrollable circumstance. The circumstances might effect the individual on an emotional, physical or criminal level. Examples can range from the death of a loved one to personal injury. Please see this report to classify the types of crisis to be reported. Once reported, your direct supervisor will follow-up appropriately, making you aware of the resolution when necessary. If you do not hear of the resolution and have personal concern regarding the situation, please follow-up with your direct supervisor.

Duty/On-Call Change Request Form
To be submitted by 3 p.m. on Thursday of the week preceding the requested change. Both staff members participating in the switch of duty night must personally submit a duty change request form. All duty/on call changes must receive final approval from the Area Coordinator of Residence Life.

Weekly Reports
To be submitted by all staff members on a weekly basis to their direct supervisor. The weekly report will address any resident, floor or hall concerns. In addition, this report should include information regarding floor interaction, upcoming programs and activities taking place within the floor/building.
