My Marital Status Has Changed.

Forms to consider:

    Personal Change form- if you move or your name changes

    W-4- if you move and/or want to change your allowances.

    Blue Cross Enrollment/Change Form- add or drop dependents.  Must be done within 30 days of from the date of marriage. 

    Standard Insurance Enrollment/Change Form- change beneficiary information

You will need to stop by the office to complete additional paperwork if you wish to add a person(s) to your health insurance and/or change beneficiaries.
