I am Moving.  Which forms do I need to fill out?

 

  1. W-4 -for your address change and local tax reasons. (must complete if you relocate)
  2. Personal Change Form- This is a document is the official record so King's College has record to change your address. This form will also notify us if we need to change your address with other institutions such as your health care provider.
